Request for Special Dietary Accommodations
The Edmonds School District is committed to supporting access to healthy school meals to all children, including those with disabilities and/or special dietary needs due to allergies or other limitations. The district is required to make reasonable modifications to meal(s) on a case-by-case basis to accommodate a child's documented dietary needs due to medical need, including allergies. Although we make every effort to offer a range of meal options that meet numerous dietary preferences, we are not able to meet all requests based on personal diet preference.
The request for meal modification must include:
- Food(s) to be omitted/avoided from the child's diet
- How the ingestion of the food impacts the child
- Food(s) that can be substituted or modified
Requests for meal modification must be signed by a State-recognized medical authority, a licensed health care professional authorized to write medical prescriptions in Washington. Acceptable options are:
- Medical Doctor (MD)
- Doctor of Osteopathy (DO)
- Physicians's Assistant (PA) with prescriptive authority
- Naturopathic Physician
- Advance Registered Nurse Practitioner (ARNP)

Once this completed form is received, the food service department will coordinate with food service staff, nursing staff, adminstratrive staff and Section 504 coordinators (if needed) at your student's school and will communicate with the parents/guardians about our options to meet the dietary prescription.
