A Sport Physical Evaluation Form is required for all athletes prior to their first day of try-outs and/or practices. Sport Physical Evaluation forms must be printed and signed by your healthcare provider and submitted to your school Athletic Secretary.

Middle School Wrestlers must have a healthcare provider complete a MS Weight Class Permit prior to their first contest each season.

COVID Waiver Form
For the 21-22 school year, a COVID Waiver form must be submitted to your school's Athletics Department. This must be received prior to their first practice/try-out date and can be submitted via email, scanned/faxed, or hard-copy.
COVID Consent to Test Form
To maintain the health and safety of our athletes, the Washington State Department of Health has updated their additional protocols beyond masking and social distancing which will allow high contact indoor sports to take place this school year.
Both Vaccinated and Unvaccinated Student-Athletes: As of Dec. 17, 2021, student-athletes trying out/participating in high-risk indoor sports (basketball, wrestling, cheer, and dance) must submit a Consent to Test form and participate in on-site COVID testing three (3) times per week. This includes all high-risk indoor sports at all high schools, middle schools, K-8 intramural sports, and Special Olympic programs.

K-8 (Maplewood/Madrona) AND SPECIAL OLYMPICS ATHLETICS
Electronic eligibility forms are currently not available for the K-8 and Special Olympics athletic programs. K-8 (Maplewood/Madrona) and Special Olympics athletes must submit hard-copy eligibility paperwork to their school's Athletic Department. An Informed Consent must be submitted for each sport you intend to participate in.

ASB and Sport Fee Waiver Process: Fee waivers will be granted to students who participate in the WSAC College Bound Scholarship program or qualify for free or reduced-price meal benefits and have consented to share their data with other programs within the District.
