Get a sports physical
A Sport Physical Evaluation Form is required for all athletes prior to their first day of try-outs and/or practices. Forms are valid for two years. In addition, middle school wresters must have a healthcare provider complete a weight class permit prior to their first contest each season.

Register online through FinalForms
- Registration must be completed by the parent or legal (court appointed) guardian of the student athlete.
- Parents/guardians can find instruction online:

- Sports Physical Evaluation Forms may be uploaded directly into FinalForms.
- Students are required to complete a few forms in FinalForms using their district email account as their user name.

K-8 (Maplewood/Madrona) AND Special Olympics Athletes
Electronic eligibility forms are currently not available for the K-8 and Special Olympics athletic programs. K-8 (Maplewood/Madrona) and Special Olympics athletes must submit hard-copy eligibility paperwork to their school's Athletic Department. An Informed Consent must be submitted for each sport you intend to participate in.

Washington Interscholastic Activities Association (WIAA)
All member schools follow established rules and eligibility requirements set by the WIAA. The organization also oversees league standings, RPI data, and State Championship Tournaments.

ASB and Sport Fee Waiver Process
Fee waivers will be granted to students who participate in the WSAC College Bound Scholarship program or qualify for free or reduced-price meal benefits and have consented to share their data with other programs within the District.
