ELIGIBILITY FORMS

SPORT PHYSICAL

A Sport Physical Evaluation Form is required for all athletes prior to their first day of try-outs and/or practices. Sport Physical Evaluation forms must be printed and signed by your healthcare provider and submitted to your school Athletic Secretary.

Middle School Wrestlers must have a healthcare provider complete a MS Weight Class Permit prior to their first contest each season.

HIGH SCHOOL AND MIDDLE SCHOOL ATHLETICS

High School and Middle School eligibility forms are submitted electronically through Rank One Sport

K-8 (Maplewood/Madrona) AND SPECIAL OLYMPICS ATHLETICS

Electronic eligibility forms are currently not available for the K-8 and Special Olympics athletic programs. K-8 (Maplewood/Madrona) and Special Olympics athletes must submit hard-copy eligibility paperwork to their school's Athletic Department. An Informed Consent must be submitted for each sport you intend to participate in.

CHEER AND DANCE TRYOUTS

Incoming freshmen (current 8th graders) may try out for the high school cheer and dance teams in the spring. The eligibility requirements for those current 8th graders/incoming 9th graders interested in trying out are as follows:

AMS, CPM, BTM, MMS STUDENTS:

  • Complete the required forms on Rank One:
    • Cheer Informed Consent
    • Eligibility Questionnaire
    • Athletic Emergency Information
  • If your student has a current updated Sports Physical in Rank One, this physical will be good for tryouts.
  • If your student does not have a current updated sports physical in Rank One, they will need to submit a hard copy of the required sports physical form to the HIGH SCHOOL Athletics Secretary or Athletic Director at your school.

K8 OR PRIVATE SCHOOLS STUDENTS:

ASB and Sport Fee Waiver Process: Fee waivers will be granted to students who participate in the WSAC College Bound Scholarship program or qualify for free or reduced-price meal benefits and have consented to share their data with other programs within the District.
