Immunization Requirements

Parents/Guardians have the responsibility for a child meeting immunization regulations, including foster parents. Parents should keep accurate and up-to-date immunization records for their children. Parents must submit a complete Certificate of Immunization Status (CIS) which is required for their children to attend school.

Any child attending school  in Washington State is required by law to be fully immunized consistent with the national immunization guidelines for their age/grade against the following diseases:

  • Diphtheria
  • Tetanus
  • Pertussis (whooping cough)
  • Poliomyelitis (polio)
  • Measles (rubeola)
  • Mumps
  • Rubella (German measles)
  • Hepatitis B
  • Haemophilus influenzae type b (Hib disease)
  • Pneumococcal
  • Varicella
