Starting today, Monday, March 22, we are changing the digital daily health attestation requirements for students.
While families will continue to receive a personalized health attestation link for each of their students via email on a daily basis, families only need to fill them out on days their student will attend in-person learning or on-site activities.
If families did not receive a digital health check attestation form this morning for each of their students, please contact your school for further assistance. It is important to make sure you are receiving the form in order to verify your email address is accurate, and that the email is not being blocked or identified as spam.

Daily Digital Health Check Attestations
The Edmonds School District is using a digital daily health check attestation system as part of our mitigation practices to keep our students, staff and families safe during the COVID-19 pandemic.
What is a digital daily health check attestation form?
It is a way for families to provide proof (the completed digital form) their student is healthy to attend school -or- that their student needs to stay home due to symptoms or exposure related to COVID-19.
Who needs to fill out the daily health check attestation form?
The district will email a digital daily health check attestation form to families each school day. Families only need to fill it out *IF* their student is coming to campus that day. Otherwise, families can disregard the email.
Which parent/guardian will receive the daily health check attestation form in their email?
The health check attestation form will only be sent to Guardian One of Family One in Skyward Family Access. Please make sure you have the correct email address listed in Skyward Family Access for the parent or guardian who you would like to receive the daily email.
Can 7th-12th grade students fill this form out themselves?
Yes. Per Snohomish Health District guidelines, families with students in 7th grade through 12th grade can choose to give their student permission to complete the daily health check attestation form themselves. In order to do this, the parent must enter a student mobile phone number to receive text messages. Students will then receive a daily text message with a link to the health check attestation form, which they can fill out on their mobile device. So to clarify, each day the parent/guardian would get an email and the student would get a text message, and whoever fills it out first - that is the data that will be collected.
What about students who live with different parents at different times? How can both parents/guardians receive the daily email?
Guardian One of Family One in Skyward Family Access will receive the daily email. For a second household, we suggest you follow the steps to have the daily text message sent to a second family, if applicable. This would mean both households would receive the daily health check attestation form, one via email, the other via text.
What about childcare before school, after school or all day care?
The district is in partnership with Right At School, Kids Krew and the YMCA to provide before, after and all day childcare services.
Families enrolled in these programs will fill out the same daily health check attestation form for their childcare provider as the district is using for all students and staff. Families participating in childcare need to fill out the daily form each morning for before school childcare. If you are only participating in after school care, the form you fill out for your student to attend school that day will be shared with your childcare provider. The goal is for families to only need to fill it out once a day.

Sí.
