Viking Athletics
Meadowdale Middle School offers 9 sports during 4 different seasons. We play the other middle schools of the Sno-King Middle School league. This league includes schools from Mukilteo, Shoreline, and the Edmonds School Districts.
To participate, students must have a current physical, an athletic eligibility questionnaire/emergency info form, and a single sport informed consent form. They also must meet the academic requirements and be in good standing regarding attendance and discipline.

1) Rank One Online Clearance Forms: Rank One Clearance Forms (all 3 forms must be submitted) plus COVID form below.
2) Signed ESD Covid Waiver Form
3) Updated Sports Physical: Make sure the athletics office has an updated copy of your sports physical on file.
**Wrestlers must have a weight form filled out by the doctor.**
Reminder: Sports Physicals are good for 2 years
Physicals can be submitted in person or can be faxed to our main office at (425) 431-7741 or emailed to Amy McVay .
4) Required Health Forms:
Students with expired Health Forms from the previous school year will need to submit updated forms to your school nurse BEFORE they will be allowed to try out for a fall sport.
The following ESD Health Forms are required for students who have a life-threatening allergy or need medication administered at school: Allergic Reaction Medication Authorization and Authorizatio
5) Sports fees are $65.00 per sport unless you are on free and reduced lunch. Students will also need an ASB card $15.00 each school year. All fees can be paid online on the district's InTouch website. If you are on free and reduced lunch please contact the Main Office for a waiver. There is an activity bus after practices for students who need it.

WINTER SEASON #1
