Step 2 & 3: Documentation and Forms
Before completing enrollment, please collect required documentation (Step 2 below). Families can then choose to register in one of two ways:
- Online enrollment which includes a fillable form (Step 3 below) and an online enrollment application (Step 4)
- Paper enrollment by completing the full enrollment packet. These packets can be found on the Paper enrollment page. Completed paper packets are then submitted directly to schools
Step 2: Gather Documentation
- Proof of residency (utility or other bill works just fine)
- Certification of Immunization Status (go to myirmobile.com to download for your student)
- If applicable - previous school information including:
- Report card or transcripts
- Withdrawal grades
- Attendance/discipline
- IEP/504 (if applicable)
- Proof of age (birth certificate or passport)
Step 3: Complete online forms packet
The online form packet includes:

Student Residency Verification Form (P-135)

Housing Questionnaire (if applicable) (P-160)

Student Health Enrollment Form (HS-534)

Certificate of Immunizations (HS-518)
Once completed with "Forms Packet," save to your computer. You will need to upload at the end of step 4.
