Student Enrollment Checklist:

• Confirm enrollment eligibility: Is your student at least age 5 by August 31 of the year entering school? View this chart to determine grade level.

• Gather required documents: Student's Proof of Birth, Proof of Residency and Student's Immunization Records. Scroll down for more details about these documents. 

• Complete Online Forms Packet:  Download this packet, complete it, and upload when you complete the online enrollment form via Skyward.

• Complete Online Enrollment through Skyward and wait for enrollment confirmation from the school

Student Enrollment Checklist

Required Documents

Whether you enroll online or in-person, you must provide the following documents. 

1. Student's Proof of Birth: Birth certificate, passport, religious, hospital, or physician’s certificate showing date of birth, an entry in a family bible, an adoption record, an affidavit from a parent, a previously verified school records, or any other documents permitted by law.
2. Proof of Residency in the parent/guardian's name: Rental/Lease Agreement or a current utility bill (gas, electricity, water, sewage)
3. Student's Immunization Records: Physical Vaccination Record or Doctor's Office/Clinic printout. (go to myirmobile.com to download your student’s immunization status)
