Following
are guidelines for equipment repair.
When sending out equipment for repair/servicing, please
fill out a Request for Hauling B160. On the form, please
put the following:
TO: where the equipment is going (please
see below)
FROM: your site (so they know where
to return the equipment) and phone number
CURRENT DATE:
UNDER DESCRIPTION: Please explain what
is wrong with the equipment, what the equipment is doing
or not doing, please be specific. After the form is
complete, tear off the Goldenrod, keep for your records,
secure the B160 on the piece of equipment, and place
the piece of equipment for Warehouse to pickup. If you
do not receive your equipment back within a week, you
may want to call the repair facility to find out the
status.
