These forms are available as paper copies at schools, as well as on this website to download and view using Adobe Acrobat. If you do not have Adobe Acrobat, you can download Adobe Reader for free. If you are enrolling your child in the Edmonds School District and are unsure which school he/she should attend, please contact the District's Transportation Office at 425-431-7230.


Athletics High School Eligibility Forms
  Athletic Clearance for Single-Sport Season (AD-1HS)
  High School Athletic Forms Packet (AD-2) (Revised 8/3/09) **MUST BE COMPLETED EACH SCHOOL YEAR**
  Athletic Survey — Optional for filling out after each sport season by parent and student.

Athletics Middle School Eligibility Forms
  Athletic Clearance for Single-Sport Season (AD-1MS)
  Middle School Athletic Forms Packet (AD-3) (Revised 8/3/09) **MUST BE COMPLETED EACH SCHOOL YEAR**
  Athletic Survey — Optional for filling out after each sport season by parent and student.

Application for Attendance in Nonresident District (P-44)
This form is for students requesting to attend school in a district other than their home district.

Authorization for Administration of Oral and Auto-injectible Medication at School (H-145)
This form should be completed by parents/guardians whose children must take medication while at school. Portions of the form must be completed by the child's licensed health professional.

Do Not Release Directory Information (P-190)
The District is allowed, and in some circumstances required by law, to release directory information unless parents/guardians or secondary students instruct us not to release this information. Directory information can include a student's name, photograph, address, telephone number, and other information. If you do not want student directory information released at all (complete privacy) or specifically to the media and/or military, complete the form and return it to the District.

Elementary and Secondary Transfer Application (P-138)
This form must be completed for students who would like to attend an elementary or secondary school other than their neighborhood school.

Internet and Electronic Communication Exclusion (IM-131)
This form must be completed only if the parents/guardians do not want their child to have access to the Internet while at school.

Parent/Guardian Informed Consent for Field Trip (T-103)
General information, medical information, and medical release are the components of this form, which should be completed and returned to school before all field trips.

Private Transportation for School Activities (T-104)
This form must be completed by individuals who are providing private transportation to children during a school-sponsored event.

Request for Distribution of Information to Students: Outside Group/Organization (A-63)
This form must be completed by nonprofits that would like to distribute fliers directly to students. Fliers must promote or contain information regarding an activity or event of possible educational, social, or recreational value or benefit to students in the district.

Student Emergency Release Authorization (P-139)
This form must be completed annually to identify the authorized person(s) who can pick up students in case of an emergency. Once completed, it should be returned to the school.

  Secondary form (P-134s)

Completing either the elementary or secondary forms and the main attachment form is the first step to register for school. **Other forms and requirements will be addressed upon visiting the school's main office with these completed registration forms.
