School Board Action Forms for Field Trips

SCHOOL BOARD ACTION FORM for FIELD TRIPS
PROCEDURES FOR SUBMITTING MATERIALS

Agenda Items — All agenda items needing Board approval must be in the Superintendent’s office at noon on the Wednesday prior to each of the scheduled meetings. Please use the Agenda Quick paperless system for all agenda items except field trips.

Field Trips needing Board approval — Under Field Trip Policy 7350, and Regulation 7350R1 the School Board is required to approve Type II and III trips. These filed trips are overnight or out-of-state trips, outdoor education programs, and international travel. Please refer to the policy and guidelines for additional information. Each trip classification has a specific set of guidelines that are to be followed.

In addition, field trip forms (.doc) should be sent via inter-school mail to the supervising assistant superintendent for input into the Agenda Quick paperless system. They will route to Risk Management before being placed on an agenda. The process for field trips includes:

  • Obtaining Field Trip Forms for Board approval — Please download the one-sheet Field Trip Request (.doc) form from this site. Information in all the boxes needs to be complete before approval will be considered.
  • All materials should be clearly legible (preferably word processed) and complete with all the information and required signatures.
  • The form must be signed by both the originator and building principal/designee.

Resolutions — If you are submitting an agenda item that requires a Resolution, please contact JoAnn Kerns (7003) to obtain a number.

School Board Meeting Dates — These are on the district calendar and on the website. They are typically held two Tuesdays each month beginning at 6:30 p.m. at the ESC.
